Alleve Wellness Clinic ("AWC", "we") is a health clinic at 8655 Weston Rd, Unit 1, Vaughan, ON L4L 9M4. You can reach us at 647-689-3207 or allevewellnessclinic@gmail.com.
What we collect on this website
- When you use our contact form: your name, phone number, email and what you'd like help with.
- When you book online: booking is handled by Jane, our clinic software. Jane's own privacy policy applies to what you enter there.
- Basic website data, such as pages visited, through our hosting provider.
How we use it
- To reply to your request, book appointments and send reminders.
- To send forms or receipts to your benefits plan or auto insurer when you ask us to.
- We never sell your information.
Text messages
If you tick the text-message box, we may text you about your request and appointments. Message frequency varies and message and data rates may apply. Reply STOP to opt out or HELP for help at any time.
Health information
Your health records are kept in Jane and handled under Ontario's Personal Health Information Protection Act (PHIPA). Only the people caring for you, and the insurers you authorize us to share them with, can see them.
Your choices
You can ask to see, correct or delete the information we hold about you by emailing allevewellnessclinic@gmail.com. Some health records must be kept for a set time by law.

We'll post any changes to this policy on this page with a new date.
